We are seeking a skilled and patient-focused

Clinical Pharmacist

to join our healthcare team. The ideal candidate will collaborate with physicians, nurses, and other healthcare professionals to ensure the optimal use of medications for patient care. This role involves assessing patient needs, optimizing drug therapy, and providing education to both healthcare professionals and patients. Key Responsibilities :

Evaluate and manage patient medication therapy to ensure safety and effectiveness. Provide expert consultation to physicians and healthcare teams on drug selection, dosages, interactions, and potential side effects. Develop and implement medication therapy management programs. Educate patients on the proper use of medications and potential side effects. Monitor patient progress and adjust medications as needed to improve outcomes. Ensure compliance with hospital policies, regulations, and best practices in pharmacotherapy. Conduct research and stay updated with the latest developments in pharmacology and clinical guidelines. Collaborate with healthcare professionals to develop treatment plans tailored to individual patient needs. Participate in clinical rounds and multidisciplinary team meetings. Train and mentor pharmacy staff, interns, and students. Required Qualifications :

Education :

Doctor of Pharmacy (PharmD) Strong knowledge of pharmacology, drug interactions, and disease management. Excellent communication and patient counseling skills. Ability to work in a fast-paced healthcare environment. Strong analytical and problem-solving skills. Attention to detail and commitment to patient safety.
