The Patient Outreach Specialist serves as a liaison between healthcare providers and patients, aiming to enhance patient engagement, improve health outcomes, and ensure continuity of care. This role involves contacting patients to schedule appointments, provide education, follow up on care plans, and assist with care coordination.
Key Responsibilities
Conduct outbound calls to patients for appointment reminders, follow-ups, and preventive care outreach.
Educate patients on available health services, wellness programs, and treatment plans.
Assist in coordinating care between departments, including scheduling tests, specialist referrals, and other services.
Update and maintain accurate patient records in electronic health systems.
Identify barriers to care (transportation, language, financial, etc.) and connect patients with appropriate resources.
Track outreach outcomes and report on key performance indicators (KPIs).
Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
Qualifications
High school diploma or equivalent; associate or bachelor’s degree in healthcare, public health, or related field preferred.
1-2 years of experience in a healthcare setting, customer service, or community outreach.
Strong communication, interpersonal, and organizational skills.
Familiarity with electronic medical records (EMR / EHR) systems.
